What documents do I need for a Debility claim?
- Completed Debility claim form
- Identity document, Birth certificate ( if minor) or Passport of the policyholder & Life Assured
- Proof of bank account for the Claimanti. Stamped bank statement
ii. If proceeds are payable into an account other than the collection account - Proof of hospitalisation (if applicable)
