Maturity claim
What documents do I need for a Maturity claim?
Completed Maturity claim form
Identity document, Birth certificate ( if minor) or Passport of the Policyholder and claimant
Proof of bank account details
i. Bank statement stamped by bank; or ii. Pay slip with full account number
Proof of residence of the Policyholder showing the physical home address. (Not a document from Sanlam), (e.g. municipality bill or lease/rental agreement or letter from the local councillor)
The above should not be older than 3 months
