Hospital claim
- What documents do I need for a Hospital claim?
Completed Hospital claim form
Identity document, Birth certificate ( if minor) or Passport of the Life Assured and Policyholder if different
Proof of bank account details for the claimant if proceeds are to be paid into an account other than the premium paying account
i. Bank statement stamped by bank; or ii. Pay slip with full account number iii. The above should not be older than 3 months.
Hospital invoice that shows the dates that the Life Assured was in hospital, reason for hospitalisation and patient’s name
