You can now submit your Funeral claim directly through our call centre. Simply call us and have the following information ready:
Personal details of the deceased
BI1663-number (This is a South African form used to notify Home Affairs of death or stillbirth, also called a Notice of death form / DHA-1666 form Once submitted Home Affairs will provide you with a BI1663 number. This number is on the top right corner of the BI 1663 document which is provided by the forensics pathologist at Hospitals where death was confirmed. This document must be completed by the doctor who confirmed the death as well as the funeral parlour)
Bank account details for claim payment. Our friendly consultants will guide you through the process.
- What documents do I need for natural case of death?
Completed Funeral claim form
Identity document, Birth certificate ( if minor) or Passport of the deceased and claimant
Certified copy of death certificate of the deceased
BI-1663 or DHA-1663 or BI-1680 from your funeral parlour or home affairs
Proof of bank account details into which the claim will be paid, or a bank statement stamped by bank
Payslip with full account number
The above should not be older than 3 months
Fully completed police report, if the cause of death is unnatural, accidental or suicide
