What documents do I need for a Disability claim?
- Completed Disability claim Medical report form
- Identity document, Birth certificate ( if minor) or Passport of the policyholder or Life Assured
- Boarding letter reflecting the date and reason (e.g. medically unfit) from employer
- All medical records submitted to employer when applying for medical board

Track your claim status
